For an appointment always bring your:

• valid passport or EU identity card for identification. A driver's license is not suitable.
• pilot's license (if applicable)
• overview of the flight hours since the last medical and in total
• previously issued medical certificate
• overview of used medication
• any contact lenses (with holder and fluid for removal) and/or glasses with spare glasses with prescription from a recent eye test if any changes have been made
• specialist or GP reports for any relevant medical conditions
